SUMMARY
Under RN supervision; provides services as assigned in the home of clients; performs related work as required per care plan.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Includes the following, other duties may be assigned.

  1. Instructs clients on how to prepare a shopping list for food, utilizing principles of basic nutrition, budgeting, buying and considering preferences of the clients
  2. Basic shopping for essentials
  3. Reports changes or situations observed in family or individual served
  4. Provides personal care such as bathing, toileting, skin and mouth care, taking and recording vital signs
  5. Assistance with self administration of medications
  6. Completes required documentation on time and accurately

QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Knowledge of home management, including housekeeping methods and techniques and household budgeting; knowledge of basic elements of nutrition and meal planning, including special diets.

EDUCATION and/or EXPERIENCE
High school graduate or equivalent preferred. Individual must be able to read, write, and communicate effectively, following instructions, and complete records and reports accurately. At least six (6) months prior experience in either the acute, long term care or community based care setting preferred or certification from approved training course.

MATHEMATICAL SKILLS
The ability to add and subtract two digit numbers and to multiply and divide with 10′s and 100′s. The ability to perform these operations using units of American money and weight measurement, volume and distance.

REASONING ABILITY
Ability to apply common sense understanding to carry out simple one or two step instructions. Ability to deal with standardized situations with only occasional or no variables.

REPORTS TO WORK AS SCHEDULED AND IS REGULAR IN ATTENDANCE
Arrives in a timely manner without excessive absences and follows procedures for proper notification of absences/tardiness. Reports to work with a clean, well groomed appearance.

PROMOTES A DESIREABLE WORK ENVIRONMENT/ATTITUDE AMONG FELLOW STAFF MEMBERS
Conveys respect for staff and clientele. Maintains a positive attitude at all times.

DISPLAYS A DEGREE OF FLEXIBILTY IN WORK ASSIGNMENTS
Acts as a team member and assists other team members to meet work requirements. Maintains a flexible schedule to meet the organizations work requirements. Acts as a preceptor to assist with new employee orientation.

ATTENDS IN-SERVICE PROGRAMS TO FULFILL THE RESPONSIBILITIES OF THE POSITION
Attends the minimum number of hours of employment related in-services/continuing education per year. Seeks in-service programs to increase competency in the job requirements specific area and of practice and/or expertise.

ACCEPTS PATIENT ASSIGNMENTS AND DISPLAYS FLEXIBILITY WITH SCHEDULE
Reviews schedule weekly and accepts PRN assignments or duties as directed  by scheduler, nurse or supervisor.

CERTIFICATES, LICENSES, REGISTRATIONS
Must be listed on the NC Nurse Aide registry if employed as a Nurse Aide. If employed as a personal care aide, eviden e of successful completion of an aide competency evaluation program is required.

The employee must frequently lift and/or move up to 50 pounds. specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

The work environment characterisitics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee is regularly exposed to outside weather conditions. The noise level in the work environment is usually quiet.
